To make an initial appointment, please call Dr. Schottenstein's office manager, Judy, at (614) 470-5582, extension 3. This extension provides all the new patient information.
Follow-up Appointment or Prescription Request
To make a follow-up appointment or request a prescription, please call Dr. Schottenstein's office manager, Judy, at (614) 470-5582, extension 2 and leave a message. You will receive a return call by day's end regarding scheduling requests. Prescriptions will also be called in by the end of the day.
Payment Information
Payment is expected at the time of service. Payment can be made by cash, check or through Venmo or Zelle. We are not set up for credit card, debit card, or health savings account card transactions.
Dr. Schottenstein is not a participating provider in any health insurance plan and does not file health insurance claims, but will provide all the information necessary in order for you to file a claim for out of network benefits.

Please note that patients are responsible for the full cost of appointments if cancellations are not received 24 hours prior to the scheduled appointment.
